Booking an online appointment in advance is REQUIRED. Please select from the available dates & times below. Each person getting the vaccine is required to have their own appointment! Don't worry, you can add multiple people to an appointment (please choose "add patient" after adding the first person). We highly suggest booking soon as spots fill quickly. Please present receipt, ID, and insurance card upon arrival.
